Some people have questions about obtaining a medical exemption from being vaccinated against COVID-19 from their Nurse Practitioner. Here is what you need to know:
• All of our Nurse Practitioners strongly recommend you be fully vaccinated against COVID-19 (with two doses of vaccine) if you are able

• Medical exemptions will not be provided except for the reasons listed below and authorized by the Ontario Ministry of Health

• Exemptions may be permitted in the following situations:

  • On the advice of an allergist/immunologist after a documented severe allergic/anaphylactic reaction to a component of a COVID-19 vaccine;
  • If you are 12-17 years old and currently being followed by a specialist for myocarditis;
  • On the advice of an allergist/immunologist after a documented severe allergic reaction or anaphylaxis following a COVID-19 vaccine where the allergist/immunologist determines there are no other options for receiving a 2nd dose of that or any other COVID-19 vaccine;
  • Following an episode of confirmed myocarditis or pericarditis after a COVID-19 vaccine if it was diagnosed by an Emergency Room physician or relevant specialist
  • Following an episode of possible myocarditis or pericarditis after a COVID-19 vaccine if a relevant specialist doctor has concluded the individual is unable to receive any COVID-19 vaccine
  • Following a serious adverse event after receiving a COVID-19 vaccine (eg requiring hospitalization or causing persistent significant disability/incapacity) which has been medically assessed and discussed with a relevant specialist doctor (allergist/immunologist, Medical Officer of Health, etc) regarding risks/benefits of potential options for immunization with the same or an alternative COVID-19 vaccine; or relevant specialist has determined that there are no options for receiving the COVID-19 vaccine
  • You are actively receiving monoclonal antibody therapy OR convalescent plasma therapy for the treatment or prevention of COVID-19
