The 360 Degree Nurse Practitioner – Led Clinic (NPLC) is a dynamic community-based interdisciplinary primary health care clinic providing comprehensive primary health care services to individuals and families residing in Peterborough City and County who are unattached to other primary care providers.

 

The NPLC is committed to providing access to innovative, integrated primary health care services and programs delivered collaboratively with a focus on healthy equity and patient centeredness. Located in the downtown core of Peterborough, we have expertise providing care to people whose health is made vulnerable by poverty, homelessness and poor access to the social determinants of health. We operate from a harm reduction philosophy. We are committed to working from within an anti-oppressive and anti-racist philosophical framework that is trauma-informed and which values cultural humility and inclusiveness.

 

Working within the interdisciplinary team, the Registered Nurse (RN) provides high quality, full scope professional nursing care to patients, consistent with the vision and philosophy of the NPLC and with the standards of the College of Nurses of Ontario. The RN will participate with the interdisciplinary team in the provision of primary health care, health promotion and disease prevention.

 

We are looking for a Nurse who:

  • Is passionate about working with people who are vulnerable: living in poverty; insecurely/un-housed; living with mental illness +/or using drugs; LGTBQ2S+; refugee; identifies as Indigenous

  • Believes in the value of client-centred, team-based care and enjoys working in an environment that empowers all members of the team, including the client, to contribute in meaningful ways
  • Has knowledge of relevant primary care practices and guidelines and means to adapt this knowledge to support marginalized populations.

 

Role and responsibilities to include:

  • Contributes to organizational activities which support the achievement of the NPLC’s Mission, Vision and Strategic Plan
  • Provide comprehensive collaborative primary care and services working within the full scope of practice as defined by the College of Nurses of Ontario
  • Womb to tomb primary care:
    1. Well-Baby Visits
    2. Chronic Disease Management
    3. Injection Clinics
    4. Wound Care
    5. Patient education
    6. Triage
    7. Immunizations across the lifespan
    8. Foot Care
  • Participate in 360NPLC programs/projects, as appropriate, which may include:
    1. Daily in-clinic nursing
    2. Providing nursing support at the Consumption and Treatment Site
    3. Outreach nursing
    4. Home Visits with other members of the 360 NPLC team
    5. Learning from and supporting the Safe Supply Project team
  • Understand how to incorporate harm reduction practices in a clinical setting
  • Practice from a trauma-informed, strengths-based, anti-oppressive lens
  • Focus on social and structural determinants of health that may influence the client’s health status
  • Support clients in being at the centre of their care, empowering them to make decisions on their health and goals and supporting patients managing their health and care
  • Participate in chart reviews, discussion of difficult cases, and client inquiries
  • Advocate for patients with colleagues, agencies, and institutions
  • Address stigma and reduce barriers to health and social care access wherever possible
  • Assure standards of patient care are maintained in accordance with accepted current best practice guidelines and 360NPLC policy.
  • Support the interdisciplinary team in ordering supplies/vaccinations, and IPAC adherence

 

Education

  • Maintain professional development through continuing education, including learning opportunities provided at the 360NPLC.

 

 

Administrative

  • Participates in designing, delivering and evaluating programs for the NPLC that are aligned with the strategic directions of the clinic and coordinated with all members of the health care team, and which address population health needs and current gaps in service for the target population
  • Participate in huddles and program, team, and general staff meetings
  • Adhere to CNO standards of practice regarding documentation, scope of practice, reflective practice and communication
  • Collect data as required for statistical/reporting purposes

 

 

QUALIFICATIONS

  • Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses of Ontario
  • Two to five years nursing experience preferably in primary care/public health/community care/mental health & addictions
  • Experience working with communities and individuals who face barriers accessing health care due to illness and other factors known to be the determinants of health
  • Experience working from harm reduction and trauma informed philosophies
  • Experience with crisis identification and de-escalation
  • Exemplary oral and written communications and interpersonal skills, along with the ability to prioritize tasks and demands of a busy program
  • Ability to work interdependently in an interdisciplinary team environment
  • Ability to effectively collaborate and communicate within an interdisciplinary team
  • Experience with electronic medical records systems is an asset
  • Satisfactory criminal reference check/vulnerable sector check

PRIMARY HEALTH CARE NURSE PRACTITIONER – (Locum, 6-month contract)

Responsibilities

  • Provide comprehensive collaborative care and services working within the full scope of practice as defined by the College of Nurses of Ontario
  • Work in collaboration with the inter-professional team to provide primary health care to manage the needs of the NPLC’s patients

 

Qualifications

  • Registration in the RN, Extended Class in good standing with the College of Nurses of Ontario
  • Minimum 2 years of primary care experience working in community-based health care settings
  • Demonstrated strong primary care clinical assessment skills to full NP scope of practice
  • Exemplary oral and written communication and interpersonal skills
  • Experience working with vulnerable populations (such as people who are homeless, living on low income, newcomer, refugees, trans individuals)
  • Experience working with people experiencing mental health or substance use issues from a harm reduction philosophy
  • Experience with electronic medical records system is an asset
  • Demonstrated ability to advocate for patients to ensure health care system responsiveness
  • Satisfactory criminal reference check/vulnerable sector check
  • Current BCLS
  • Current Ontario Driver’s License
