Organization Overview

The 360 Degree Nurse Practitioner – Led Clinic (360 NPLC) is a dynamic community-based interdisciplinary primary health care clinic providing comprehensive health care services to individuals in Peterborough City and County, with particular expertise in providing care to people whose health is made vulnerable by poverty, homelessness and poor access to the social determinants of health. We operate from a harm reduction philosophy. We are committed to working from within an anti-oppressive and anti-racist philosophical framework that is trauma-informed and which values cultural humility and inclusiveness.

Working as part of the interdisciplinary team, the Registered Nurse (RN) delivers high-quality, full-scope nursing care in alignment with the 360 NPLC’s vision and philosophy and the standards of the College of Nurses of Ontario. The RN contributes to the delivery of primary health care and actively supports health promotion and disease prevention initiatives alongside the team.

In this role, the RN will work collaboratively across all programs delivered by the 360 NPLC, including providing comprehensive care across the lifespan within primary care, as well as supporting the Consumption and Treatment Site at 220 Simcoe St. by facilitating access to health services and working closely with the Harm Reduction team to monitor and assist service users.

Role and responsibilities to include:

  • Contributes to organizational activities which support the achievement of the NPLC’s Mission, Vision and Strategic Plan
  • Provide comprehensive collaborative primary care and services working within the full scope of practice as defined by the College of Nurses of Ontario
  • Lifelong primary care, relevant to the needs of the individual and programs
  • Well-Baby Visits
  • Chronic Disease Management (Diabetes, COPD, Mental Health, Substance Use)
  • Injections (B12, Long Acting Injectables, immunizations)
  • Wound Care
  • Triage
  • Immunizations across the lifespan
  • Foot Care
  • Phlebotomy
  • Patient education and health promotion based on individual needs
  • Care coordination, system navigation, and advocacy
  • Home visits or Outreach as needed
  • Participate in 360NPLC programs and projects, as appropriate
  • Understand how to incorporate harm reduction practices in a clinical setting
  • Practice from a trauma-informed, strengths-based, anti-oppressive lens
  • Focus on social and structural determinants of health that may influence the client’s health status
  • Support clients in being at the centre of their care, empowering them to make decisions on their health and goals and supporting patients managing their health and care
  • Participate in chart reviews, discussion of difficult cases, and client inquiries
  • Advocate for patients with colleagues, agencies, and institutions
  • Address stigma and reduce barriers to health and social care access wherever possible
  • Assure standards of patient care are maintained in accordance with accepted current best practice guidelines and 360 NPLC policy.
  • Support the interdisciplinary team in ordering supplies/vaccinations, and IPAC adherence
  • Maintain professional development through continuing education, including learning opportunities provided at the 360 NPLC
  • Participate in huddles and program, team, and general staff meetings
  • Adhere to CNO standards of practice regarding documentation, scope of practice, reflective practice and communication
  • Collect data as required for statistical/reporting purposes

QUALIFICATIONS

  • Registered Nurse in good standing with the College of Nurses of Ontario
  • Two to five years of nursing experience with priority given to candidates who have experience in primary care, public health, community care, or mental health & addictions
  • Experience working with communities and individuals who face barriers accessing health care due to physical or mental illness or poor access to the social determinants of health.
  • Experience working from harm reduction and trauma informed philosophies
  • Experience with crisis identification and de-escalation
  • Exemplary oral and written communications and interpersonal skills, along with the ability to prioritize tasks and demands of a busy program
  • Ability to work interdependently in an interdisciplinary team environment
  • Ability to effectively collaborate and communicate within an interdisciplinary team
  • Experience with electronic medical records systems is an asset
  • Satisfactory criminal reference check with vulnerable sector check
  • Lived experience with substance use, mental illness, and/or homelessness is valued as it can provide enhanced perspective in supporting clients
