Organization Overview
The Peterborough 360 Degree Nurse Practitioner – Led Clinic (NPLC) is committed to providing access to innovative, integrated primary health care services and programs delivered collaboratively with a focus on healthy equity and patient centeredness. Located in the downtown core of Peterborough, we have expertise providing care to people whose health is made vulnerable by poverty, homelessness and poor access to the social determinants of health. Position Title: Registered Practical Nurse Hours per week: 22.5
Job Summary
Working within the interdisciplinary team, the Registered Practical Nurse (RPN) provides high quality professional nursing care to patients, consistent with the vision and philosophy of the NPLC and the standards of the College of Nurses of Ontario. The RPN collaborates with the interdisciplinary team in the provision of primary health care, health promotion and disease prevention.
Key Areas of Responsibility
• Conduct individual patient assessments including patient history, recording of vitals and biometric measurements
• Share results of assessment and interventions with the NPLC team as appropriate
• Implement various goal-oriented, time-limited intervention plans, including education in collaboration with patient and/or family and inter-professional team
• Provide client follow up and patient education as directed by the NPs or other IHPs
• Provide culturally sensitive care
• Perform point of care testing (urine, HCG, rapid strep throat) under medical/NP directives
• Provide wound care within the RPN scope of practice
• Provide therapeutic injections
• Assist in the maintenance of a clean, orderly work environment
• Arrange priorities as necessary to perform tasks
• Document nursing care clearly, concisely and in a timely manner
Qualifications
• Current certificate as a Registered Practical Nurse (RPN)
• Registration with College of Nurses of Ontario in good standing
• Registration with the RPNAO or other proof of professional liability protection
• Minimum 3-5 years related experience in community nursing, preferably in community-based primary care nursing
• Demonstrated strong primary care clinical assessment skills to full RPN scope of practice
• Experience with health promotion and disease prevention education and programming
• Demonstrated ability to navigate the health care system on behalf of patients
• Demonstrated ability to advocate for patients to ensure health care system responsiveness
• Current Basic Cardiac Life Support certification
• Experience performing venipunctures an asset
• Experience with electronic medical records system is an asset
Position Title: Medical Receptionist Hours per week: 15
Job Summary
As the first point of contact for clients, the receptionist role is vitally important. The receptionist will support the primary care providers in the provision of primary health care through compassionate, professional clerical support. The receptionist’s primary role is to provide direct and indirect contact with clients, community partners, visitors and to liaise and provide clerical support to the primary care providers.
Key Areas of Responsibility and Duties:
• Performing reception-related duties in a compassionate, professional, nonjudgmental and helpful manner
• Ability to accommodate multiple demands and to prioritize work
• Responding to inquiries from telephone calls, visitors, staff; booking appointments; managing the daily schedule for all providers
• Managing incoming and outgoing faxes, referrals, emails and prescriptions
• Photocopying and distributing materials; handling incoming mail and lab results
Qualifications
• Excellent customer service and interpersonal skills
• At least 5 years of work experience in a front-line capacity in a health care setting
• Excellent information technology skills including a working knowledge of Windows, Microsoft Office Programs (Excel, Word, Outlook and PowerPoint) and Internet searches
• Demonstrated ability to multi-task
• A diploma or certificate in Office Administration Medical Secretary program an asset
• Experience working with an electronic medical record for scheduling an asset
General Qualifications for 360 NPLC Staff
• Excellent written and verbal communication and organizational skills
• Demonstrated ability to work independently and in a team environment
• Experience working with vulnerable populations individuals who face multiple challenges gaining access to services including those who have experienced trauma (such as people who are homeless, living on low income, newcomers or refugees)
• Experience working with people experiencing mental health or substance use issues, and with harm reduction as an approach to substance use
• Experience with crisis de-escalation
• Ability to work evenings or weekends if required
• Ability to maintain confidentiality of personal health information
• Demonstrated ability to work independently and in a team environment
• Satisfactory criminal reference check with vulnerable sector check
